Patient Care Manager  Responsibilities:  The Patient Care Manager (PCM) will implement, coordinate, and supervise all aspects of the delivery of patient/family services, consistent with policies and procedures and State and Federal regulations.  The PCM is responsible for the supervision, coordination, and delivery of direct patient/family care services and for the interdisciplinary team component of the program.  Responsible for supervision and evaluation of RN Case Managers, Socials Workers, Chaplains, Bereavement Coordinator, Volunteer Coordinator, Home Health Aides, and Ancillary Staff.  Qualifications:  A Registered Nurse with a current state license to practice as a professional nurse in the state the office is located.A minimum of three years' experience as a professional nurse, including one year of full-time experience in providing direct patient care in a hospice, home health or oncology setting, and one year of supervisory experience in hospice or home health care preferred.Demonstrates ability to manage all aspects of terminal care with specific expertise in crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.Demonstrates skill in administrative performance, which includes expertise with communication, both written and oral, supervision of staff, documentation requirements and service coordination.Demonstrates ability to execute a supportive, sensitive approach to staff and patient/family needs.  Duties:  1. Assuring that nursing services delivered are provided according to established hospice program policies2. Assuring that nursing services are available 24 hours a day, 7 days a week and that licensed practical nurses and home attendants work under the direct supervision of a registered nurse3. Participating in the development and implementation of orientation and in-service training hospice programs for all levels of nursing staff employed by the hospice program4. Acting as nurse liaison with staff and other agencies, hospice programs and individuals that have contractual agreements to provide nursing services;  5. Participating in quality improvement reviews and evaluations of the nursing services provided6. Directing or supervising the delivery of nursing services7. Supervises the planning, coordination and delivery of patient/family care8. Assigns each patient/family to appropriate staff member(s), or delegates responsibility to appropriate staff9. Provides for regular interdisciplinary team meetings for the development, review, and revision of the patient/family Plan of Care, including exchange of information, review of problems or unique issues from caseloads, and sharing of professional support10. Provides for and conducts staff meetings for information and staff support, as necessary11. Makes visits to patient/families as indicated by patient/family need, staff need, or for the purpose of supervision12. Acts as a RN Case Manager during periods of understaffed situations13. Supervises the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards14. Provides verbal and written performance evaluations for assigned interdisciplinary team members15. Promotes the utilization of resources for the benefit of team members and patients/families consistent with agency procedures16. Identifies need for staff education and assists in the development of inservice programs for interdisciplinary team17. Assist in planning, delivery and evaluation of volunteer, student, bereavement and education/training programs18. Participates in staff selection and oversees orientation of new personnel19. Maintains the budget for the clinical components of the overall agency budget20. Accepts other assignments as appropriate.
