Qualifications Required

• Bachelor’s degree or higher in Physical Therapy from an accredited institution

• Minimum of 1 year of healthcare experience

• 1+ years of experience in any of the following:

  • Electronic Medical Record (EMR) systems

  • TruChart EHR

• Valid Physical Therapist license in the state of Michigan (MI)

• Valid Regular Driver’s License (DL)

Preferred: Please provide your PT license number (required for application process)

Key Responsibilities

• Manage and deliver physical therapy care plans within patients' homes

• Collaborate with interdisciplinary team members to ensure quality care delivery

• Utilize EMR systems and mobile technology for real-time charting and documentation

• Travel to patient homes and maintain timely documentation of care provided

Schedule and Shift Details

• Monday – Friday; 8:00 AM / 9:00 AM – 5:00 PM

Travel

• Local travel to residential homes (approximately 20% of the day)
