We are seeking a dedicated Primary Care Physician to join our growing team in Tyler, TX. Our innovative network of neighborhood primary care centers is designed to provide equitable, patient-centered, and value-based healthcare to local Medicare populations.
\nOur physicians are supported by multidisciplinary care teams and advanced clinical technology, allowing you to focus on what matters most—delivering quality outcomes and preventive care, not patient volume.
\nDeliver comprehensive, patient-focused primary care services within a value-based care model.
\nManage a patient panel of approximately 13–16 patients per day.
\nConduct visits averaging 20 minutes for regular appointments and 40 minutes for new or complex cases.
\nCollaborate with a dedicated care team including a Medical Assistant, Scribe, and Social Worker.
\nUtilize on-site lab services with phlebotomy support and access to RubiconMD for virtual specialty consultations.
\nMaintain accurate documentation using Canopy and Greenway EMR systems.
\nParticipate in community health initiatives such as wellness classes, exercise groups, and social engagement activities.
\nSchedule: Monday–Friday, 8:00 AM – 5:00 PM
\nClinic Model: 3–5 providers per site (mix of MDs and NPs)
\nTransportation Support: Patient pickup service within 5 miles of the clinic
\nCommunity Engagement: Regular on-site patient wellness events (yoga, dance, spa days, and more)
\nMedical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
\nBoard Certified in Internal Medicine or Family Medicine
\nActive, unrestricted State Medical License and DEA
\nMinimum of 1 year post-residency clinical experience (no residents)
\nBilingual (Spanish) preferred
\nGeriatrics Fellowship training, preferred
\nBase Salary: $215,000–$300,000+ Incentive Bonuses

Compensation:
Base Salary: $215,000–$300,000(commensurate with experience and location)
