Job Description

  • Perform advanced clinical pharmacy duties to optimize pharmacotherapy for transplant patients.
  • Conduct proactive and reactive medication regimen reviews to ensure safety and effectiveness.
  • Collaborate with healthcare professionals to manage patient drug therapies and improve care transitions.
  • Serve as a mentor and preceptor for pharmacy students and residents.
  • Participate in quality improvement initiatives, including formulary management and medication use evaluations.
  • Support innovative research and contribute to publications and presentations.

Qualifications

  • Education: PharmD from an accredited institution.
  • Experience:
  • Completion of ASHP-accredited PGY1 residency (required).
  • PGY2 residency in solid organ transplant or equivalent experience (preferred).
  • Licensure: Eligible for Texas pharmacist licensure.
  • Skills: Strong analytical and collaborative abilities, with a passion for patient-centered transplant care.
