LPN or MA - Case Manager

 

 

About the Opportunity

FC Staffing has been retained by a major healthcare system in Northern Florida — a respected network with about 50 wholly owned offices and 30 affiliate clinics — to hire a compassionate and dedicated LiveWell Case Manager.

This role is based primarily in Daytona Beach and involves supporting patients and families through a family practice outpatient environment, with occasional visits to nearby clinics to ensure continuity of care.

Location: Daytona Beach, FL — with occasional visits to nearby office locations within Northern Florida

Employment Type: Full-Time | Salary + Benefits

Schedule: Monday–Friday, 8:00 AM–5:00 PM (with flexibility for patient/family needs)

Compensation: Competitive salary based on experience, qualifications and certifications

 

Position Overview

The Case Manager serves as a vital bridge between patients, families, and the healthcare team. You will help individuals diagnosed with chronic or terminal illnesses navigate care options, provide education, and ensure their treatment aligns with their personal values and health goals.

 

Key Responsibilities

1. Patient Engagement & Education

• Serve as the primary contact for patients facing advanced or serious illnesses.

• Educate patients and families about palliative and hospice care options, coverage, and eligibility.

• Facilitate goals-of-care discussions and advance care planning.

2. Clinical Coordination & Navigation

• Collaborate with primary care providers, specialists, and care managers to ensure seamless coordination across care settings.

• Conduct patient assessments to identify symptoms, emotional needs, and support systems.

• Support transitions from hospitals, skilled nursing, or rehabilitation facilities to home-based care.

3. Advocacy & Support

• Advocate for patient-centered care emphasizing dignity, comfort, and quality of life.

• Provide emotional and educational support to patients, caregivers, and families.

• Connect families to community and faith-based resources when appropriate.

4. Documentation & Compliance

• Maintain accurate, timely, and HIPAA-compliant documentation in the EMR.

• Ensure compliance with CMS and insurance guidelines for palliative and hospice care documentation.

 

Qualifications

Required:

• Active Licensed Practical Nurse (LPN) or Medical Assistant (MA) license in Florida.

• Minimum 3 years of experience in palliative care, hospice, or case management.

• Strong communication and emotional intelligence, especially around end-of-life discussions.

• Valid driver’s license and reliable transportation for interoffice travel and patient visits.

Preferred but not required

Certification in Hospice & Palliative Nursing (CHPN), Case Management (CCM), or Advance Care Planning Facilitation.

• Bilingual (Spanish or Creole preferred).
