๐Ÿ“ Position Overview:

The Director of Case Management is responsible for leading and overseeing case management operations at the hospital level. This includes utilization management, care facilitation, discharge planning, and quality improvement initiatives. The role ensures the effective and compliant delivery of services that support both patient care and resource optimization.

๐Ÿ‘ฅ Supervises:

  • Case Managers

  • Social Workers

๐Ÿ”‘ Key Responsibilities:

  • Lead daily workflow of Case Managers and Social Workers

  • Ensure compliance in documentation and regulatory standards

  • Collaborate with finance and quality departments on program development and data analysis

  • Participate in and facilitate discharge planning and multidisciplinary rounds

  • Cover patient caseloads when needed to support department staffing

  • Communicate effectively with physicians for care planning and resource management

  • Manage hiring, scheduling, performance reviews, and payroll functions

  • Participate in Utilization Review and Revenue Recycle Committees

  • Ensure Joint Commission, CMS, and state regulatory compliance

โœ… Requirements:

Education:

  • Graduate of a Registered Nursing program (required)

  • Bachelor of Science in Nursing (preferred)

Experience:

  • Minimum 2 years in case management (e.g., utilization review, discharge planning)

  • At least 2 years in hospital-based nursing

  • Management experience (2โ€“3 years preferred)

Licenses & Certifications:

  • Current RN license (Iowa or multistate)

  • Mandatory Reporter Certification (Child & Dependent Adult Abuse โ€“ Iowa)

Knowledge & Skills:

  • Proficiency in payer regulations, Medicare, managed care, and continuum of care

  • Strong leadership, communication, and collaboration abilities

  • Working knowledge of performance improvement and clinical documentation standards
