QualificationsRequired:

• Master’s degree or higher in Physical Therapy
• Valid Physical Therapist (PT) license in Illinois (IL)
• Valid driver’s license and reliable personal transportation
• Minimum of 1 year of experience in a healthcare setting

Preferred:

• Home Health experience or Inpatient Rehab experience
• Familiarity with OASIS documentation and HomeCare HomeBase (HCHB) EMR system
• Please provide your PT license number if available

OverviewJoin our Award Winning Client’s respected Home Health team as a full-time Physical Therapist. In this pay-per-visit (PPV) role, you'll help patients regain mobility and independence from the comfort of their homes. You’ll assess physical functioning, create tailored treatment plans, deliver therapies, educate caregivers, and collaborate with other healthcare professionals—making a lasting impact in the Northern Illinois region.

Key Responsibilities

• Conduct patient assessments to develop individualized Physical Therapy treatment plans
• Document patient progress and report changes in condition
• Provide therapy services and teach patients/caregivers how to use assistive devices
• Supervise physical therapy assistants and certified nursing assistants when applicable
• Participate in staff development meetings and prepare detailed reports on patient status

Schedule and Shift Details

• Monday–Friday, day shift (start time depends on patient scheduling)
• No patients scheduled past 5:00 PM
• One weekend every 6 weeks (Saturday & Sunday)
• Two holidays per year
• Full-time expectations: 30 points/week, approximately 40 hours per week

Territory and Travel Expectations

• Local travel within Gurnee, North Chicago, Grayslake, surrounding lake areas, and up to the IL/WI border
• Mileage reimbursement typically provided

Compensation & Bonus Opportunities

• Pay-per-visit (PPV) – competitive rates vary by service type and experience
• $10,000 First-Year Success Bonus (FYS)
