Position Overview

Our client, a leader in home health services, is seeking a compassionate and experienced Home Health Medical Social Worker (MSW) to join their care team. The role involves collaborating with healthcare professionals to deliver patient care plans, supporting patients in their homes, and addressing their unique needs. This position requires a balance of clinical expertise, emotional support, and resource coordination to enhance patient outcomes.

Key Responsibilities

  • Collaborate with interdisciplinary teams to manage and deliver individualized patient care plans.
  • Support patients and families in understanding and coping with their medical, emotional, and social challenges.
  • Provide care and services to patients in Jackson and surrounding areas.
  • Conduct psychosocial assessments to identify needs and offer appropriate interventions.
  • Educate patients and families on available community resources and assist with accessing them.
  • Maintain accurate, timely, and compliant documentation in the Electronic Medical Record (EMR) system.
  • Travel to patient locations (approximately 20% of the workday), ensuring continuity of care and accessibility.

Qualifications

Required

  • Master’s Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE).
  • 1-2 years of post-graduate experience in social work, preferably in a healthcare or home health setting.
  • Valid MSW license issued by the state of Michigan, in good standing.
  • Current and valid driver’s license, reliable transportation, and auto insurance.
  • Proficiency with EMR systems (e.g., TruChart EHR) and experience using tablets and mobile devices.

Schedule and Travel Requirements

  • Day shift, Monday through Friday.
