Our Award-Winning Client is seeking an RN Care Manager to join their Care Management Team at their Santa Monica, CA Health Center. This is a full-time role with 8-hour day or variable shifts.

The RN Care Manager plays a vital role in care coordination, transitional care management, and utilization management, working collaboratively with patients, caregivers, nurses, social workers, physicians, payers, and community resources. This position ensures that patients receive the most effective and efficient care, supporting self-determination and addressing social determinants of health while navigating complex healthcare systems.

Key Responsibilities:

• Perform comprehensive patient assessments to determine appropriate care coordination and resource allocation.
• Facilitate care transitions and discharge planning, ensuring optimal patient outcomes.
• Collaborate with healthcare teams, insurance providers, and community organizations to streamline patient care.
• Implement utilization management strategies to ensure compliance with healthcare regulations and best practices.
• Advocate for patients' rights, ensuring timely access to quality care and necessary resources.
• Utilize evidence-based practices to develop personalized care plans that enhance patient recovery and wellness.

Required Skills & Qualifications:

• Bachelor’s Degree in Nursing (BSN) or equivalent education/experience.
• Active California Registered Nurse License (RN) upon hire.
• 2+ years of experience in a healthcare setting (Acute, Ambulatory, Post-Acute, etc.).
• 2+ years of experience in Case Management, Care Coordination, or Utilization Management OR successful completion of the Transitions in Practice (TIP) program for Care Managers (TIP candidates must have experience in the same type of nursing unit as the Care Manager role).

Preferred Qualifications:

• Master’s Degree in Nursing (MSN) or Healthcare-related field.
• Certification in Case Management (ACM or CCM) upon hire.

Candidate Profile:

• Experience Level: 2–5 years of relevant experience.
• Seniority Level: Associate.
• Management Experience: Not required.
• Travel Requirement: Occasional travel may be necessary.
