Qualifications

Required:

  • Doctorate degree (MD or DO)
  • 2+ years of experience as a Physician
  • 1+ years of experience in ANY of the following:
    • Geriatric Care
    • Senior Care
    • Geriatric/LTC (Long-Term Care)
  • Valid Licenses/Certifications:
    • Medical Doctor (MD) license in California (CA)
    • Drug Enforcement Administration (DEA) registration in California (CA)
    • Controlled Dangerous Substances (CDS) certification
    • American Heart Association Basic Life Support (AHA BLS)
    • Board Certification in relevant medical specialty
  • Proof of:
    • Valid California driver’s license
    • Personal transportation, a clean driving record, and auto insurance

Preferred:

  • 1+ years of PACE Experience
  • Bilingual in Spanish

Overview

Join our client's team as a Physician for PACE participants, where you will play a crucial role in delivering holistic, patient-centered care to high-risk seniors and frail older adults. Your focus will be on ensuring their well-being, independence, and quality of life through comprehensive medical care and interdisciplinary collaboration.

Key Responsibilities

  • Deliver direct primary medical care to PACE participants, including conducting history and physical exams
  • Grow and manage your own participant panel in close collaboration with a complex care RN
  • Evaluate, review, and close orders for medications, diagnostic studies, referrals, and treatments to ensure high-quality care
  • Provide care where participants need it most—in the PACE center, at home, virtually, or in skilled nursing facilities (SNFs)
  • Collaborate with IDT (Interdisciplinary Team) members, including behavioral health specialists and pharmacists, to create comprehensive care plans
  • Serve as an integral member of the IDT, completing assessments, participating in care planning, and reviewing Service Determination Requests
  • Gain exposure to Value-Based Care strategies and learn how to effectively manage high-need, high-cost populations
  • Take after-hour calls remotely on a rotating schedule, supported by a nurse triage team
  • Make clinical decisions and coordinate care across disciplines
  • Educate caregivers and manage referrals to specialists
  • Ensure accurate EMR documentation

Schedule and Shift Details

  • Monday - Friday, 8:00 AM - 5:00 PM (flexible schedule with on-call shifts)
  • Must be willing to travel to patients' homes, SNFs, ALFs, and hospitals as needed
