Our Award Winning Client is a renowned national organization that has achieved multiple accolades and operates on a wide scale. They are currently seeking a dedicated Physical Therapist to join their dynamic Home Health team. This role involves delivering Physical Therapy services in patients' homes, evaluating their functional levels, and creating customized treatment plans to enhance independence and overall physical/cognitive functioning. Additionally, you will engage in educating patients and caregivers, supervising therapy assistants, and documenting patient progress.

Qualifications

Required:

• Master’s degree or higher
• Minimum of 1 year of experience in a healthcare setting
• Valid Physical Therapist license in Illinois (IL)
• Valid Regular Driver's License (DL)

Preferred:

• PT license number provided

Key Responsibilities

• Conduct comprehensive Physical Therapy assessments and devise effective treatment programs
• Monitor and document patients' responses to therapy and any changes in their condition
• Guide patients and caregivers on therapy programs and the use of therapeutic devices
• Oversee physical therapy assistants and, as needed, certified nursing assistants
• Prepare detailed reports and contribute to staff and agency development activities

Schedule and Shift Details

• Schedule: Monday – Friday; day shift (start time varies based on patient needs); no patients past 5:00 pm; 1 weekend every 6 weeks (working Saturday & Sunday with scheduled patients) and 2 holidays per year

Travel

• Local travel to patients' homes

Compensation

• This role offers PPV rates as outlined in the attached PT PPV Rates document, plus a $10,000 hiring bonus.
• Full-time, 40 hours per week; 30 points per week
• Coverage Area: Westchester, LaGrange, Brookfield, Riverside, North Riverside, McCook, Lyons, Hillside, Broadview, Cicero, Berwyn, Stickney & surrounding areas
• IL PT license required, with at least 1 year of PT experience (no new grads); 1 year of home health experience preferred but not required (inpatient rehab experience acceptable as a substitute); OASIS & HCHB highly preferred but will be taught in orientation
