As a licensed Nurse Practitioner, you will lead the medical aspects of team-based care for individuals with serious mental illness. This position involves documenting and addressing the medical needs of individuals by performing a whole-person clinical assessment to identify and address care gaps. Ongoing responsibilities include care coordination, making referrals as needed, and providing clinical support to Guides (peers) and Community Resources Guides (social workers).

You Will:

  • Perform a whole-person clinical assessment by:

    • Gathering information from other care providers
    • Assessing behavioral and physical health through a thorough history and physical examination
    • Capturing undiagnosed conditions and ordering additional studies as indicated
    • Organizing information to support disability applications (if appropriate)
    • Conducting visits in the field or at individuals’ homes
    • Making personalized recommendations or referrals based on assessed needs
  • Lead the medical component of team-based care, including:

    • Training and advising firsthand Guides and Community Resources Guides on clinical issues and treatments
    • Supporting the Guide Team with care coordination
    • Managing care and identifying actionable care gaps
    • Performing medication reconciliation post-discharge and as needed
    • Communicating and coordinating with individuals’ care providers at CMHCs, FQHCs, and hospitals

  • Ability to maintain professional boundaries while engaging a population with challenges
  • Intermediate computer skills and familiarity with Microsoft Office Suite
  • A valid driver’s license, private auto insurance, and the ability to be insured under the company’s coverage

The Experience You Bring Includes:

  • 5+ years of clinical care experience as a Nurse Practitioner, ideally in a primary care setting
  • Experience managing underserved populations with behavioral health and/or substance use disorder (SUD) conditions
  • Experience developing and delivering educational programs for nurses and medical staff
  • Experience in care management and coordination

Compensation:

  • $142.5K Base
  • 10% Bonus Potential
  • Incentive: At 36 months, a guaranteed lump sum payment of $30K plus a $10K base salary adjustment
  • $2K allowance for continuing education and recertification
  • Normal M-F schedule with very occasional consultative weekend calls (expected to be light or non-existent)

Must Haves & Preferred Qualifications:

  • 5+ years of clinical care experience as a Nurse Practitioner, ideally in primary care
  • Strong candidates with a focus on Primary Care
  • Must be from the local community and have connections in the immediate area
  • Coding experience preferred
