Responsibilities:

Working under the guidance of the Home Health Clinical Manager, your core duties will include:

  • Conducting comprehensive nursing assessments, developing care plans, and evaluating patient progress based on physician orders and patient needs.
  • Providing professional nursing services within patients’ homes, ensuring adherence to corporate policies, procedures, and the Nurse Practice Act.
  • Managing assigned patients' care, carrying out case management duties, and ensuring continuity of care across all health services.
  • Collaborating with the interdisciplinary team to ensure high standards of patient care are met.

Required Skills and Qualifications:

  • Graduate of an accredited Nursing program with a Bachelor’s degree in Nursing.
  • Active RN license in Pennsylvania.
  • BLS Certification.
  • Valid Driver's License and Current Automobile Insurance are required for travel to patient homes.
  • Strong ability to manage and coordinate care across various settings and patient needs.
  • Proven case management experience and excellent patient communication skills.

Candidate Profile:

The ideal candidate brings 1+ to 2 years of nursing experience, preferably in home health or hospice care. This role requires someone willing to travel to patient homes and capable of adapting care to meet diverse patient needs. While management experience is not required, candidates must demonstrate the ability to work autonomously within a supportive team structure.
