Key Responsibilities:

• Conduct in-person comprehensive assessments and develop care plans in collaboration with the IDT.
• Oversee participant care in nursing homes and hospitals, managing admissions, discharges, and the coordination of acute, short-term, and long-term care.
• Collaborate with community physicians and the IDT for effective discharge planning.
• Provide primary care, continuous medical coverage, and regular nursing home visits based on participant needs.
• Participate in night and weekend call rotations.
• Offer guidance and direction to Nurse Practitioners and other IDT members.

Required Qualifications:

• Doctorate Degree (MD or DO)
• Board Certification in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine
• Active California Physician License without Restrictions
• State CDS Certification and DEA Registration
• Experience working with geriatric patients is preferred but not required.
