Position: Home Health Physical TherapistLocation: Chambersburg, PA, and surrounding areas

Schedule

  • Workdays: Monday - Friday, 8:00 AM - 5:00 PM
  • Weekends: 1 weekend/month
  • Holidays: 2 holidays/year
  • On-call Requirement: None

Compensation & Bonus

  • Pay Rate: $64-$72 per visit (Pay Per Visit, PPV)
  • Bonus Details:
  • $10,000 first-year success bonus.
  • $5,000 sign-on bonus (paid after 30 days of full-time employment).

As a Home Health Physical Therapist, you’ll create and implement personalized care plans to help patients recover and improve their quality of life. Qualifications

Required:

  • Education: Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
  • Licensure: Valid Pennsylvania Physical Therapist (PT) license.
  • Experience: Minimum of 1 year in healthcare.
  • Travel: Valid driver’s license with the ability to travel (~20% of time spent driving).
  • Technology Skills: Familiarity with EMR systems and mobile devices like tablets and smartphones.
  • Time Management: Ability to manage schedules effectively and meet charting deadlines.

Preferred:

  • Prior experience with EMR platforms (e.g., Practice Partner).
  • Submission of valid PT license number for verification.

Key Responsibilities

  • Assess and evaluate patients’ needs to design and execute personalized care plans.
  • Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
  • Travel to patients’ homes to deliver therapy sessions while adhering to safety and care standards.
  • Maintain accurate, timely documentation using EMR systems.
  • Educate patients and families on therapy goals and progress.
