This role involves collaborating to provide patient care plans, supporting patients in the area, and demonstrating skills in time management and technology use. Key Responsibilities

• Collaborate to provide management and delivery of patient care plans
• Support patients in Springfield, IL and surrounding areas
• Consistently meet critical deadlines for charting
• Self-manage time and schedule

• Traditional M-F, 8 am - 5 pm

Qualifications

Required

• Associate degree or higher
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• 1+ years of experience in ANY of the following:
  • Acute Care
  • Rehabilitative Setting
• Valid Physical Therapy Assistant license in the state of Illinois (IL)
• Valid Regular Driver License (DL)

Preferred

• 1+ years of Home Health experience
