Physical Therapist - Home Health

Overview:We are looking to expand our exceptional care team with compassionate and dedicated Physical Therapists. As a Home Health Physical Therapist, you will collaborate with a team of healthcare professionals to manage and deliver personalized patient care plans. Key Responsibilities:

• Manage and implement individualized patient care plans.
• Travel locally to provide in-home patient care.
• Document patient information accurately and promptly using Electronic Medical Record (EMR) systems.
• Meet critical deadlines for charting and reporting.
• Collaborate with the care team to ensure comprehensive patient support.

Qualifications:Required:

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Valid Physical Therapist license in the state of Pennsylvania (PA).
• Minimum of one year of healthcare experience.
• Experience with EMR systems (e.g., Practice Partner EMR or others).
• Valid Regular Driver’s License (DL).
• Ability to travel locally (~20% of the day) to patient locations.
• Excellent time management and self-scheduling skills.

Preferred:

• Submission of PT license number.

Schedule & Shift Details:

• Full-Time: Monday – Friday, 8:00 AM – 5:00 PM.
• One weekend per month & two holidays per year.
• No on-call requirements.
