Qualifications

Required:

• Associate degree or higher

• 1+ years of experience in ANY of the following:

  • Electronic Medical Record (EMR)

  • Other EMR

  • TruChart EHR

• Valid Licensed Practical Nurse (LPN) license in Pennsylvania (PA)

• Valid Regular Driver’s License (DL)

• Are you okay with the required travel for this position?

Preferred:

Overview

Our Award Winning Client is seeking a dedicated Home Health LPN to join their extraordinary care team in Pleasant Hills, PA. This full-time role offers a comprehensive benefits package and a supportive work environment. The ideal candidate will have an Associate degree, a valid LPN license in PA, and relevant experience in patient care.

Key Responsibilities

• Manage and deliver patient care plans

• Collaborate with the care team to provide high-quality patient care

• Support patients in Pleasant Hills, PA, and surrounding areas

• Meet critical charting deadlines

• Self-manage time and schedule effectively

• Utilize tablets, mobile phones, and EMR software

Schedule and Shift Details

• Schedule: Monday–Friday, 8-hour days

• Weekend: About 1 weekend per month

Location & Travel

• City/State: Pleasant Hills, PA

• Territory/Driving Expectation: 20–25 miles surrounding the posted location

• Travel: Occasional local travel

Preferred Experience:

• Prior home health (HH) or hospice experience

• At least 1 year of medical-surgical (med/surg) experience

• Minimum 1 year of nursing experience

Excluded Experience:

• Private duty, dialysis, or other limited-scope areas

Compensation

• Pay Rate: Around $30/hr (salaried role)
