Our Award Winning Client, a nationally recognized leader in healthcare innovation and compassionate patient care, is currently seeking an experienced RN Care Manager to join their dedicated Care Management Team at their high-performing medical center in Tarzana, CA.

As an RN Care Manager, you’ll play a pivotal role in promoting efficient, patient-centered care by facilitating communication and collaboration across all members of the healthcare delivery team. This role combines elements of care coordination, transitional care, and utilization management, ensuring patients receive optimal support across the continuum of care.
You will contribute to a team responsible for ensuring effective transitions, timely resource utilization, and personalized care plans that recognize both the medical and social aspects of each patient’s needs.
Work Schedule: 8-hour variable shifts with potential hours from 8:00 AM–4:30 PM or 4:30 PM–1:00 AM and weekend rotations.
Coordinate care and services across the continuum to promote timely and efficient care delivery
Facilitate the development and implementation of individualized care plans in collaboration with patients, caregivers, and clinical teams
Evaluate patient needs related to discharge planning, community resources, and long-term care options
Participate in utilization management functions to support appropriate resource use and reduce unnecessary costs
Advocate for patients while balancing safety, quality, and regulatory requirements
Engage in interdisciplinary rounds and case conferences to drive care goals
Identify and address barriers to care and communicate effectively across departments and with external providers
Bachelor’s Degree in Nursing OR enrollment in a BSN program with completion within 3 years of hire (exceptions may be considered)
Valid California RN License upon hire
Valid California Fire and Life Safety Card (within 30 days of hire)
Minimum 2 years of healthcare experience in acute, ambulatory, or post-acute care settings
Minimum 2 years of case management experience (Care Coordination or Utilization Management)
OR successful completion of the Transitions in Practice (TIP) program with unit-specific experience
Master’s Degree in Nursing or healthcare-related field
Certification in Case Management (ACM or CCM) upon hire
2 to 5 years of progressive healthcare experience
Bachelor's degree or higher (or in progress as noted)
Background in collaborative, multidisciplinary environments
Passionate about patient advocacy and outcomes
Willing to work variable shifts and weekends as required
